Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with Illinois members, developing personalized care plans and coordinating services to improve health outcomes and independence.

You will meet members in the field—at home, in nursing facilities, or a location of their choice—assessing goals, barriers, and resources, and linking them to HCBS and other supports through an interdisciplinary approach.

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or related field.
• 2 years of experience with Medicare/Medicaid recipients, LTSS, HCBS, or managed care organizations.
• 1+ year of experience with custodial and transitional care.
• Experience using electronic information applications/software programs including electronic medical records.
• Ability to travel regionally to meet members and care teams.

Responsibilities

• Provide LTSS support focusing on health-related social needs.
• Build relationships with facility staff and advocate for member care, including access to behavioral health services.
• Contact members to establish goals, evaluate resources, develop plans of care, and identify LTSS providers and community partnerships.
• Use an interdisciplinary approach to access long term services and supports regardless of funding sources.
• Develop and modify Individual Service Plans with the care team.
• Assist members through navigation of LTSS and related environmental and social needs.
• Utilize information pertaining to members to prevent duplicative assessments.
• Make recommendations for Home and Community-Based Services to enable independence.
• Facilitate interactions with payer sources, providers, and interdisciplinary care teams.
• Educate members on maintaining Medicaid eligibility.
• Enter annual Level of Care assessment into state portal.
