Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work in Illinois, meeting members in homes or facilities to assess needs and develop personalized care plans. The role requires Illinois residency, 2+ years in Medicare/Medicaid-related services, and the ability to travel regionally.

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, or related field.
• Two years of experience working with Medicare/Medicaid recipients, long-term care services, HCBS, or managed care organizations.
• One year or more of professional experience working with members in custodial and transitional care.
• Experience using a variety of electronic information applications/software programs including electronic medical records.
• Ability to travel in the region to meet face-to-face with members and/or their families, community partners and care teams.

Responsibilities

• Meet members in the field at home, NF, or location of the member's choice and assess goals and needs.
• Develop individualized care plans and connect members with LTSS and community services.
• Advocate for member care and coordinate with facility staff and care teams.
• Identify LTSS providers and partnerships to address member goals.
• Educate members to maintain Medicaid eligibility and navigate HCBS.
• Assist with entry of annual Level of Care assessments into state portals.
