Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with members in Illinois, meeting them in their homes or facilities to create personalized care plans and coordinate services. You will promote independence by addressing health-related social needs and connecting them with LTSS, HCBS, and community resources.

The role requires field travel (~75-90%) and collaboration with care teams, facility staff, and family members to drive person-centered outcomes and ensure Medicaid

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or a related field.
• Two (2) years of experience working with Medicare and Medicaid recipients, long-term care services, HCBS, or managed care organizations.
• One (1) or more years of professional experience working with members in custodial and transitional care.
• Experience using electronic information applications including electronic medical records.
• Ability to travel regionally to meet members, families, partners, and care teams.

Responsibilities

• Provide specialized support for LTSS with focus on health-related social needs and ensuring LTSS meets the member's service needs.
• Build relationships with facility staff and advocate for member care, including access to behavioral health services.
• Contact members to establish goals, evaluate resources, develop plans of care, and identify LTSS providers and community partnerships.
• Develop and modify Individual Service Plans with care team involvement.
• Assist with navigation of LTSS, environmental and social needs, and access to community services.
• Educate members on maintaining Medicaid eligibility and related processes.
