Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work in the field with members, developing personalized care plans and coordinating long-term services to improve health outcomes. The role requires extensive travel within Illinois and occasional in-office meetings in Schaumburg.

You will build relationships with facility staff, advocate for member care, and collaborate with care teams to address social needs, housing, and HCBS access, while guiding families through Medicaid

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or a related field.
• Two (2) years of experience with Medicare/Medicaid recipients, HCBS, or managed care organizations.
• One (1) year or more of experience with custodial and transitional care.
• Experience using electronic information applications including EMRs.
• Ability to travel regionally to meet members, families and care teams.

Responsibilities

• Meet members in the field at home, NF, or member location to assess goals and plan care.
• Coordinate services and connect members with LTSS, HCBS and community resources.
• Develop plans of care and involve care team members.
• Navigate LTSS and related social needs across providers and payers.
• Educate members to maintain Medicaid eligibility and access benefits.
• Support with entry of Level of Care assessments in state portals.
