Job summary

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or a related field.
• Two years of experience with Medicare/Medicaid recipients, long-term care services, HCBS, or managed care organizations.
• One or more years of experience with custodial and transitional care.
• Experience using EMR and other electronic information systems.
• Ability to travel regionally to meet members and families.

Responsibilities

• Provide LTSS-focused support for members, addressing health-related social needs.
• Advocate for member care with NFs, including access to behavioral health services.
• Conduct field visits to assess goals and develop plans of care with LTSS providers.
• Coordinate with care teams and community partners to deliver services.
• Educate members to maintain Medicaid eligibility.
• Enter annual Level of Care assessments into state portals.
