Job summary

Humana is seeking a Transition/Custodial Prevention Specialist to work with members in Illinois to create personalized care plans, coordinate services, and promote health outcomes and independence.

You will meet members in the field at home, Nursing Facility, or other locations, assessing goals and barriers and connecting them with quality services and supports. This role requires 75-90% field travel and a 40-hour work week.

Qualifications

• Must reside in Illinois.
• Bachelor's degree in social sciences, social work, human services, or related field.
• Two years of experience with Medicare/Medicaid recipients or LTSS.
• Experience with HCBS or managed care organizations.
• Ability to travel regionally to meet members and families.

Responsibilities

• Meet members in the field at home, Nursing Facility, or location of the member's choice.
• Develop individualized care plans and connect members with LTSS providers.
• Coordinate services and supports to promote independence and better health outcomes.
• Collaborate with facility staff and care teams to support member goals.
• Educate members on Medicaid eligibility and navigate state programs.
• Maintain documentation and use EMR systems.
