Job summary

Humana Inc. is seeking a Transition/Custodial Prevention Specialist to work remotely in Illinois. You will meet members in the field, assess needs, and develop personalized care plans to promote independence and better health outcomes.

The role focuses on LTSS and health-related social needs with community partnerships and an interdisciplinary care approach. You will travel regionally to visit members and participate in care planning, ensuring access to Medicaid/HCBS services while maintaining

Qualifications

• Bachelor's degree in social sciences, social work, human services.
• Two (2) years of experience working with Medicare and Medicaid recipients, long-term care services, HCBS, or managed care organizations.
• One (1) or more years of professional experience working with members in custodial and transitional care.

Responsibilities

• Meet members in the field at home, NF, or location of the member's choice.
• Assess goals, needs and barriers; develop plans of care and LTSS providers.
• Coordinate services and supports to promote independence and better health outcomes.
• Educate members to maintain Medicaid eligibility and navigate LTSS options.
• Coordinate with care teams and community partners to optimize services.
