Elevance Health seeks an LTSS Service Coordinator - RN Clinician to manage member care plans across the health continuum. This field-based role travels to client sites within a defined region, providing hands‑on care coordination and telephonic/face‑to‑face assessments to optimize outcomes.

The ideal candidate holds an active RN license, at least 3 years working with chronic illnesses or disabilities, and aligns with state contract requirements.

Qualifications

• HS diploma or equivalent required.
• 3+ years experience with chronic illnesses/disabilities in case management or similar roles.
• Current, active RN license in applicable state(s).
• State certification may be required by state law/contract.
• Associates/MA/MS in Health/Nursing preferred.

Responsibilities

• Manage member cases within licensure scope and develop care plans to optimize health across the care continuum.
• Conduct telephonic or face‑to‑face clinical assessments for identification, evaluation and coordination of member needs.
• Provide direction to non‑RN clinicians (LPN/LVN, LSW, LMSW, etc.) and oversee care coordination tasks.
• Coordinate authorizations/referrals and interface with Medical Directors/Physician Advisors as needed.
• Collaborate with members, families, and care teams to set short and long‑term goals.
