Job summary

Elevance Health is hiring a LTSS Service Coordinator Clinician who will operate primarily in the field, traveling to client sites and occasionally attending meetings at the office. The role supports the RN in assessing and coordinating services, including physical and behavioral health, social services, and long-term supports.

The position requires licensure in related fields (LPN/LVN, LSW, LCSW, LMSW or equivalent) and a minimum of 2 years’ experience with chronic illnesses.

Qualifications

• Current, unrestricted state license (LPN/LVN, LSW, LCSW, LMSW or other) required.
• Minimum 2 years of experience with chronic illnesses in a service coordination role.
• May require state-specific certification per state law/contract.

Responsibilities

• Assist the RN in identifying members at high risk of complications.
• Obtain clinical data as directed by the RN.
• Coordinate care with members with chronic illnesses and disabilities to optimize health benefits usage.
• Provide information to the RN for care plan development and modification as needed.
