Job summary

Elevance Health seeks a LTSS Service Coordinator – Clinician to serve field-based clients across Tennessee. You will identify high-risk members, collect clinical data, and coordinate with RNs to develop and monitor care plans tailored to chronic conditions and disabilities.

Qualifications

• Requires licensure as LPN/LVN or social work license and related credentials.
• Minimum two years of experience with chronic illnesses, co-morbidities, or disabilities in a service coordination role.
• Experience coordinating care with RNs and other health professionals.

Responsibilities

• Assists in identifying members for high risk complications.
• Obtains clinical data as directed.
• Assists in identifying members that would benefit from alternative levels of care or waiver programs.
• Provides information to the RN who develops and monitors the care plan.
• Coordinates care for members with chronic illnesses to ensure efficient use of health benefits.
