Job summary

Elevance Health is seeking an LTSS Service Coordinator - RN Clinician to oversee member care plans and coordinate services across medical and social supports. This field-based role travels to client sites and may attend meetings or training at an office location.

The RN will work under licensure, directing non-RN clinicians and ensuring comprehensive care planning with emphasis on coordinating health, behavioral health, social services, and long-term supports.

Qualifications

• HS diploma or equivalent is required.
• Minimum of 3 years of experience with individuals with chronic illnesses, co-morbidities, or disabilities in a Service Coordinator/case management role.
• Current, active, valid and unrestricted RN license in applicable state(s).
• State-specific certification may be required by law or contract.

Responsibilities

• Manage member's case within licensure scope; develop and monitor care plans.
• Perform telephonic or face-to-face assessments for health, behavioral health, social services, and long-term services.
• Provide direction to non-RN clinicians and coordinate services within benefits or contracts.
• Interface with Medical Directors and care teams on treatment plans and issues.
• Coordinate with non-RN clinicians, assign tasks, review performance and ensure care plan adequacy.
