Job summary

CVS Health Corporation is seeking a Case Manager Registered Nurse – Field in New Jersey. The role involves up to 75% field travel within Essex County and surrounding counties, with a Monday–Friday 8am–5pm EST schedule.

You will develop and implement care plans, coordinate with providers, and ensure cost-effective use of resources for Managed Long-Term Services and Supports and Dual Special Needs Program members.

Qualifications

• Must reside in Essex & Surrounding Counties, NJ.
• Active and Unrestricted RN license in NJ.
• 3+ years clinical practice experience.
• Willingness to travel up to 75% within Essex and surrounding counties in New Jersey.
• NJ Choice certification completion is a contingency of employment with an exam score of 80% or higher.

Responsibilities

• Conduct face-to-face visits in members' homes using assessment tools for MLTSS and D-SNP/FIDE to determine medical needs and coordinate services.
• Coordinate and advocate for proper discharge planning with the care team.
• Develop care plans with the member and authorize services in a cost-effective manner within MLTSS/FIDE benefits.
• Document accurately in the member's electronic health record in a timely manner.
• Mentor new hires and collaborate with PCP and care team to ensure integrated care.
