Qualifications

• Must reside in Essex and/or surrounding counties in New Jersey.
• 3+ years clinical practice experience in hospital or equivalent settings.
• Active and Unrestricted RN license in NJ.
• Willing to travel up to 75% in NJ with mileage reimbursement.
• NJ Choice certification completion required.

Responsibilities

• Conduct face-to-face visits in members' homes for assessments in MLTSS/ D-SNP programs.
• Develop care plans and authorize cost-effective services.
• Collaborate with PCPs and care team; document in EHR.
• Mentor new hires after proficiency achieved.
