CVS Health Corporation is seeking a compassionate and experienced Registered Nurse Case Manager to support members in Essex County, NJ and nearby areas. This is a remote role with up to 75% field travel, Monday–Friday, 8:00 AM–5:00 PM EST.

Qualifications

• Active, unrestricted RN license in New Jersey.
• 3+ years of clinical practice experience.
• 3+ years in home health, hospice, or similar setting.
• Residency in Essex County NJ.
• NJ Choice Certification with required components.
• Willing to travel up to 75% with mileage reimbursed.

Responsabilités

• Conduct in-home, face-to-face assessments for MLTSS and D-SNP/FIDE.
• Evaluate needs and coordinate services such as day care, personal care, nursing facility placement, and MLTSS enrollment.
• Develop and implement individualized care plans with members, caregivers, PCPs, and teams.
• Authorize services per guidelines and support safe transitions of care.
• Document all case activity accurately in the electronic health record.
• Apply clinical judgment to manage complex care needs.
• Mentor new team members once proficiency is achieved.
