Job summary

CVS Health Corporation is seeking a Case Manager, Registered Nurse (Field-Based) to support members across Essex County and surrounding areas. This remote role requires up to 75% field travel, with hours 8:00 AM–5:00 PM EST, Monday through Friday.

You will coordinate care, develop individualized plans, and ensure safe transitions of care with PCPs and interdisciplinary teams. Qualified candidates hold an active NJ RN license, 3+ years of clinical and home health experience, NJ Choice

Qualifications

Active, unrestricted RN license in New Jersey.
3+ years of clinical practice experience.
3+ years of home health, hospice, or similar setting experience.
Ability to travel up to 75% with reliable transportation.
Residency in Essex County, NJ.
NJ Choice Certification requirements: minimum 80%, state training modules, field mentoring.

Responsibilities

Conduct in-home, face-to-face assessments for MLTSS and D‑SNP/FIDE.
Evaluate member needs and coordinate services (medical day care, personal care, nursing facility placement, MLTSS enrollment).
Develop and implement individualized care plans with members, caregivers, PCPs, and interdisciplinary teams.
Authorize services per program guidelines and support safe transitions of care.
Document all case activity accurately and promptly in the EHR.
Apply clinical judgment and critical thinking to manage complex care needs.
Mentor new team members after proficiency is achieved.
