Job summary

CVS Health in New Jersey is seeking a Registered Nurse Case Manager to coordinate care for members with complex needs in home and community settings. This field-based role offers flexibility to work from home when not in the field, with up to 75% travel and a schedule Mon-Fri 8:00-5:00 PM EST.

You'll conduct in-home assessments, develop personalized care plans, and collaborate with interdisciplinary teams to promote safety and quality of life.

Qualifications

• Active, unrestricted RN license in New Jersey.
• Minimum 3 years of clinical experience.
• NJ Choice Certification completed with 80%+ score.
• Residence in Morris County or nearby.
• Willingness to travel locally.

Responsibilities

• Conduct in-home assessments for MLTSS members.
• Evaluate medical, behavioral, and social needs.
• Collaborate with providers and care teams to coordinate holistic care.
• Develop and manage individualized care plans.
• Document assessments in the EHR.
• Support care transitions and connect to community resources.
• Mentor new team members.
