Job summary

CVS Health Corporation in New Jersey is seeking a compassionate Registered Nurse Case Manager – Field to coordinate care for members with complex needs in Monmouth County and surrounding communities. This field-based role offers a balance of independent work and meaningful community service.

You will travel up to 75% within your county, conduct in-home assessments, develop care plans, collaborate with providers, and mentor new team members.

Qualifications

• Active, unrestricted RN license in New Jersey.
• 3+ years of clinical experience (hospital, home health, ambulatory care).
• Residence in Monmouth County or nearby communities.
• Willingness to travel frequently and visit patients in-home.
• Strong assessment, communication and critical-thinking skills.
• Completion of NJ Choice Certification and related requirements.

Responsibilities

• Conduct in-home assessments for members in MLTSS programs.
• Evaluate medical, behavioral, and social needs to support care.
• Collaborate with providers, caregivers, and teams for coordinated care.
• Develop and manage individualized care plans to promote safety and quality of life.
• Participate in care team meetings and advocate for member needs.
• Document assessments and activities in the electronic health record.
