Job summary

ISYS Case Management in Fremont, CA is seeking an experienced RN case manager to provide field-based services for workers’ compensation. You will coordinate care, attend appointments, and communicate with carriers, providers and employers to optimize outcomes.

Qualifications

• High School Diploma required.
• Current valid State Driver’s license.
• Registered Nurse (RN-BC).
• Excellent English language skills: speaking, writing, grammar, professional communication.
• Proficient with PCs, Windows and MS Office Suite; able to type at moderate speed.
• Strong prioritization, problem solving, organization and time management.
• Dependability, honesty, determination and compassion.
• Willingness to obtain CCM certification after the first year.
• Willingness to establish home office with strong internet connection.
• Understanding and appreciation of customer service; ability to serve.
• All credentials and licensure must be valid and maintained.

Responsibilities

• Provide field-based case management in workers’ compensation, coordinating care with carriers, providers, vendors, employers and attorneys.
• Attend medical appointments in-person; drive to service areas; ensure timely documentation and cost containment.
• Coordinate and manage caseload autonomously; schedule and prioritize activities.
• Conduct onsite case management following CMSA and ISYS guidelines.
• Facilitate physician appointments, hospital visits, case conferences, home visits and ancillary visits to support treatment recommendations.
• Engage medical providers to discuss diagnosis, treatment and prognosis.
• Support return-to-work goals and progression toward Permanent and Stationary designation.
