Job summary

ISYS SOLUTIONS INC is seeking a field-based RN Case Manager to support injured workers in San Jose, CA and surrounding areas. You will coordinate medical care, communicate with insurers and providers, and attend appointments in person, ensuring timely outcomes.

The role requires RN-BC licensure, strong English skills, and willingness to travel. Compensation includes salary and bonus with flexible scheduling and a robust benefits package.

Qualifications

• RN-BC licensure required and valid
• High School Diploma required; Bachelor's degree or higher preferred
• Excellent English communication skills (spoken/written)
• Strong time management, organization and problem-solving abilities
• Willingness to obtain CCM certification after first year
• Ability to work remotely from home office with reliable internet
• Credentials must be valid and maintained

Responsibilities

• Provide field-based case management in the workers' compensation space.
• Attend medical appointments and coordinate care with doctors, insurers, and employers.
• Travel within designated service areas to in-person visits.
• Maintain timely and accurate documentation; ensure CMSA and ISYS guidelines are followed.
• Communicate proactively with all parties to enable optimal recovery and return-to-work outcomes.
• Manage caseload autonomously with proactive scheduling and prioritization.
