Job summary

Somatus in New York (Queens area) seeks an RN Care Manager, VBC to support high-need CKD/ESKD patients. You will collaborate with physicians, health teams, and patients to implement individualized care plans and improve outcomes.

Role involves home visits, clinic visits, and education across settings, with travel up to 75% within the assigned region. You will work in a physician-practice partnership, coordinating care across the care continuum.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years RN experience in a multi-disciplinary team with physicians.
• BLS certification from a licensed AHA or Red Cross facility.
• Renal, CKD or Dialysis care experience as a main focus.
• Reside where high-speed internet is available or can access it.
• Able to travel to partner hospitals, clinics and community facilities.

Responsibilities

• Partner with physicians and practice staff to improve patient outcomes.
• Coordinate regular interdisciplinary care team meetings with practices.
• Establish positive relationships with patients and provider offices.
• Develop partnerships to support clinical and operational goals.
• Collaborate to optimize workflows with provider practices.
• Educate provider practices on the Somatus program and workflows.
• Provide comprehensive care via in-person, telehealth or phone visits.
• Travel to member homes, facilities and offices for visits and planning.
• Oversee non-licensed and licensed team members, delegating tasks.
