Job summary

Somatus is seeking an RN Care Manager to lead care coordination for high‑needs CKD/ESKD patients. You’ll partner with physicians and providers, perform home visits, and develop individualized care plans to improve outcomes across clinical and community settings.

Travel up to 75% within your assigned region, collaborate in a multidisciplinary team, and support transitions of care with education and telehealth touchpoints to ensure seamless, patient‑centered care.

Qualifications

• RN license and ability to license in other states as needed.
• 2+ years of RN experience in a multi-disciplinary team with physicians.
• Valid BLS certification from an approved facility.
• Renal CKD or dialysis care experience as a main focus.
• Residency in a location with high-speed internet or access to reliable service.
• Willingness to travel to partner hospitals, clinics, and facilities within your region.

Responsibilities

• Outreach and scheduling face-to-face visits with members in homes, dialysis clinics, or physicians’ offices.
• Develop and implement individualized care plans with nephrology teams.
• Coordinate with providers to improve patient outcomes and reduce readmissions.
• Educate patients on discharge requirements and self-management.
• Lead and participate in care management meetings with interdisciplinary teams.
