Job summary

Somatus is seeking an RN Care Manager to support CKD/ESKD patients through in-person and telehealth visits, coordinating with nephrology and primary care teams. You will develop individualized care plans, educate patients, and travel within your assigned region to ensure seamless care transitions.

The role requires RN licensure, 2+ years in a multi-disciplinary team, and travel flexibility. A BSN and CCM certification are preferred qualifications for career growth within Somatus.

Qualifications

• RN license with ability to obtain licenses in other states as needed.
• 2+ years RN experience in a multi-disciplinary team.
• BL S certification from an approved facility.
• Renal, CKD, or dialysis care experience as a focus.
• Resident with reliable high‑speed internet or access to it.
• Willingness to travel locally to partner hospitals, clinics, and facilities.

Responsibilities

• Partner with physicians and practices to improve patient outcomes.
• Develop and lead regular interdisciplinary care team meetings.
• Conduct in-person, telehealth, and home visits for high‑risk patients.
• Educate patients on discharge care requirements and renal condition.
• Coordinate care plans with nephrologists, PCPs, and care teams.
• Support transitions of care and reduce readmissions.
