Job summary

Somatus is seeking an RN Care Manager to support high‑need CKD/ESKD patients through home visits, clinic visits, and coordination with nephrology and PCPs. You will educate patients, build care plans, and drive outcomes in a field-based role across assigned markets.

Ideal candidates have an RN license, 2+ years' multidisciplinary team experience, and willingness to travel. Travel and flexible scheduling enable proactive patient care and collaboration with care teams.

Qualifications

• RN license valid and able to be licensed in other states.
• 2+ years RN experience in a multi-disciplinary team.
• BLS certification from a licensed facility (AHA/Red Cross).
• Renal CKD or Dialysis care experience.
• Willingness to travel within assigned region regularly.

Responsibilities

• Coordinate home, clinic, and facility visits for high-risk patients.
• Partner with nephrologists and PCPs to optimize care plans.
• Educate patients on discharge and renal care requirements.
• Support care management meetings with provider practices to improve outcomes.
• Assist in transitions of care to reduce readmissions.
• Guide both non-licensed and licensed team members in care coordination.
