Job summary

Somatus is seeking an RN Care Manager to coordinate care for high‑need CKD/ESKD patients in the field, visiting members at home, dialysis clinics, and physician offices. You’ll build trusted relationships with nephrology practices and collaborate with the interdisciplinary team to improve outcomes.

Travel within your assigned region is required, with telehealth and in‑person visits, comprehensive assessments, and care planning designed to reduce readmissions and remove barriers to care.

Qualifications

• RN license and ability to get licensed in other states as needed.
• 2+ years of RN experience, including working as part of a multi-disciplinary team and Physicians.
• Valid BLS certification from an AHA or American Red Cross facility.
• Renal, Chronic Kidney Disease or Dialysis Care experience as main focus of your job.
• Residence near reliable high‑speed internet and ability to travel within the region.

Responsibilities

• Outreach and face‑to‑face visits in homes, dialysis clinics, and physician offices to support high‑risk members.
• Collaborate with nephrology practices to improve patient outcomes and care coordination.
• Develop individualized care plans and provide education to patients and families.
• Participate in regular interdisciplinary care team meetings and telehealth sessions.
• Support transitions of care and reduce readmissions through proactive follow‑ups.
