21st Century Home Health Services is seeking a Physical Therapist Case Manager, Level 2, to provide skilled services in patients' homes in the Vallejo, CA area. You will evaluate, treat, and manage care plans for clients while supervising PTAs and coordinating with physicians.

The role emphasizes autonomy, flexible scheduling, territory-based assignments, and access to MedBridge CEU resources, with a strong focus on quality outcomes and patient independence.

Qualifications

• Current CA Physical Therapist license is required.
• Experience supervising PTAs in home-health settings is preferred.
• Strong communication and teamwork skills are essential.

Responsibilities

• Perform home evaluations and develop care plans with PTAs.
• Help patients with mobility and regain independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients' physicians to ensure quality care.
• Evaluate and report patient progress to the physician.
• Provide concierge support with documentation and admin tasks.
• Offer flexible scheduling and territory-based assignments.
