Job summary

21st Century Home Health Services (21HHS) is seeking a Full Time Physical Therapist Case Manager, Level 2 to serve the Rohnert Park, CA area. You will evaluate patients in their homes, address mobility needs, and restore function to maximize independence.

You will supervise 2 PTAs, coordinate with physicians, and manage care plans and documentation using our electronic systems. A competitive benefits package applies, with flexible scheduling and mileage reimbursement.

Qualifications

• Must hold current CA Physical Therapist License.
• Experience in home health care is highly preferred.
• Knowledge of local community resources is required.

Responsibilities

• Assess and treat patients in their homes using prescribed PT programs.
• Supervise 2 PTAs and ensure performance standards.
• Develop care plans and coordinate with physicians for quality care.
• Create detailed treatment and care plans for each patient.
• Manage flexible scheduling and territory assignments to optimize visits.
