21st Century Home Health Services (21HHS) is seeking a Full Time Physical Therapist Case Manager, Level 2, to provide in-home physical therapy services in the Hayward, CA area. You will evaluate patients in their homes, develop and implement care plans, and supervise PTAs to restore function and maximize independence.

The role requires an unrestricted CA PT license, BLS/CPR certification, and strong knowledge of local resources.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a Home Health environment is a plus.
• Knowledge of local community resources.

Responsibilities

• Evaluate and treat patients in their homes following prescribed PT programs.
• Supervise PTAs and ensure performance standards.
• Create detailed treatment and care plans for each patient.
• Coordinate with physicians to ensure quality of care.
• Provide concierge administrative support as needed.
• Work with a flexible schedule and targeted territories to minimize travel time.
