Job summary

21st Century Home Health Services in the San Jose area is seeking a Full Time Physical Therapist Case Manager, Level 2. You will evaluate patients in their homes, create and implement therapy plans, and collaborate with a team of clinicians to restore function and help patients live safely at home.

Responsibilities include supervising two PTAs, coordinating with physicians, and guiding patients toward independence.

Qualifications

• CA license to practice physical therapy in California.
• Experience supervising PTAs in a home health setting preferred.
• Current BLS/CPR certification.
• Valid California driver's license and reliable vehicle.
• Knowledge of local community resources for patients.
• Experience with high-acuity patients and interdisciplinary teams.

Responsibilities

• Evaluate patients in home settings and develop care plans with PTAs.
• Restore function and promote independence.
• Supervise 2 PTAs and ensure performance standards.
• Coordinate with physicians to ensure quality care.
• Document progress and adjust treatment plans as needed.
