Job summary
21st Century Home Health Services seeks a licensed Physical Therapist Case Manager, Level 2, to provide skilled home-based PT services in the Vallejo, CA area. You will evaluate patients, develop care plans, and supervise PTAs to restore function and independence while coordinating with physicians for high-quality care.
The role requires in-home visits, CA licensure, CPR/BLS, and a valid driver’s license. Competitive benefits and flexible scheduling are offered.
Qualifications
- Current unrestricted CA Physical Therapist License.
- 1+ year experience supervising PTAs in a HH environment is a plus.
- Current BLS/CPR Certification.
- Valid CA Driver's License.
- Knowledge of local community resources and CA home health regulations.
- Experience with high acuity patients and interdisciplinary teams is preferred.
Responsibilities
- Perform in-home evaluations and develop care plans with PTAs.
- Administer physical therapy to restore function and independence.
- Supervise 2 PTAs to ensure performance standards.
- Coordinate with patients' physicians to ensure quality care.
- Report patient progress to physicians and MD.
