Job summary

21st Century Home Health Services Inc. is seeking a Physical Therapist Case Manager, Level 2, to provide in-home therapy and supervision of PTAs in the Vallejo, CA area. You will evaluate patients, design care plans, and work with physicians to ensure high-quality care.

Responsibilities include coordinating care, supervising PTAs, and documenting patient progress. A CA PT license and BLS/CPR are required; home health experience is a plus. Competitive compensation and benefits are offered.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a HH environment is a plus.
• Current BLS/CPR Certification.

Responsibilities

• Perform home evaluations and develop a care plan; supervise PTAs to follow the plan.
• Help patients regain mobility and independence via targeted therapy.
• Create detailed treatment and care plans for each patient.
• Coordinate with physicians to ensure quality care.
• Evaluate and report progress to the MD.
