21st Century Home Health Services in Castro Valley, CA seeks a Physical Therapist Case Manager, Level 2, to provide in-home skilled PT services. You will evaluate patients, create care plans, supervise PTAs, and coordinate with physicians to ensure high-quality care.

Requires CA PT license, CPR, and valid driver’s license; remote work is not typical. Eligible candidates will join a leading Bay Area home health team with strong outcomes and employee support.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in a HH environment is a plus.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources and homecare guidelines.

Responsibilities

• Perform an evaluation in the home and develop a care plan; work with PTAs to follow the plan.
• Help patients with mobility, movement, and regain independence.
• Create detailed treatment and care plans for each patient.
• Communicate and coordinate with patients’ physicians to ensure quality care.
• Evaluate and report patients’ progress to MD.
