Job summary

21st Century Home Health Services is seeking a Full Time Physical Therapist Case Manager, Level 2, to provide skilled services in patients' homes in the Vacaville, CA area. You will evaluate and treat patients, develop care plans, supervise 2 PTAs, and coordinate with physicians to ensure high-quality care while promoting independence.

Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year supervising PTAs in HH.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Experience with high-acuity patients is a plus.
• Experience with an interdisciplinary team is desired.
• Self-directed, empathetic with strong interpersonal skills.
• Leadership and organizational abilities.

Responsibilities

• Perform home evaluations and develop care plans with PTAs.
• Assist patients with mobility and independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with physicians to ensure quality care.
• Evaluate progress and report to MD.
• Provide documentation support and administrative tasks.
