Job summary

21st Century Home Health Services seeks a Full Time Physical Therapist Case Manager, Level 2 in American Canyon, CA to evaluate patients in their homes and address mobility needs. You will lead a small team of PTAs, develop and implement care plans, and coordinate with physicians to deliver high-quality home health services.

The role requires a CA license, BLS/CPR, and the ability to work in patients' homes with flexible scheduling and strong interpersonal skills.

Qualifications

• Current unrestricted CA Physical Therapist License
• 1+ year experience supervising PTAs in a HH environment
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources and health care regulations

Responsibilities

• Perform home evaluations and develop care plans with PTAs to follow.
• Supervise PTAs to ensure they meet performance standards.
• Treat and evaluate patients using prescribed physical therapy programs.
• Create detailed treatment and care plans for each patient.
• Coordinate with patients' physicians to ensure quality care and report progress.
