Job summary

21st Century Home Health Services is seeking a Physical Therapist Case Manager, Level 2, to serve the Vacaville, CA area. You will evaluate patients in their homes, treat and address needs to restore function and independence.

You will supervise 2 PTAs, coordinate with physicians, and design individualized care plans. A CA PT license and current BLS/CPR are required, with flexible scheduling and a robust benefits package.

Qualifications

• Current CA Physical Therapist License required.
• 1+ year experience supervising PTAs in HH environment preferred.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Experience with high-acuity patients a plus.
• Ability to work independently and with an interdisciplinary team.

Responsibilities

• Perform an evaluation in the home and develop a care plan with PTA guidance.
• Help patients regain mobility and independence.
• Create detailed treatment and care plans for each patient.
• Coordinate with physicians to ensure quality care.
• Evaluate and report patients’ progress to MD.
• Supervise 2 PTAs to ensure performance standards.
