Qualifications

• Current unrestricted CA Physical Therapist License.
• 1+ year experience supervising PTAs in home health.
• Current BLS/CPR certification.
• Valid CA driver's license.
• Knowledge of local community resources and interdisciplinary collaboration.

Responsibilities

• Evaluate and treat patients in home health following physician orders.
• Develop and adjust in-home care plans with PTAs and clinicians.
• Coordinate care with physicians to ensure high-quality outcomes.
• Document progress and report to MD; supervise PTAs to meet standards.
• Support mobility and independence for patients in their homes.
