Humana is seeking a Care Coordinator to support Illinois Medicaid Members within the MLTSS/LTSS programs. You will assess needs, develop personalized care plans, and connect families with resources, with extensive travel across local settings.

Responsibilities include coordinating services, monitoring care plans, and assisting enrollees with grievances and appeals. The role requires IL residency and relevant healthcare licensure or degree, with travel 75-90% and a Monday–Friday schedule.

Qualifications

• Bachelor's degree in social sciences, social work, human services, or related field.
• Unrestricted LPN in Illinois with one year experience conducting comprehensive assessments and provision of formal service for the elderly.
• Licensed Professional Counselor (LPC).
• Licensed Social Worker (LSW).

Responsabilités

• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long-Term Care Facilities and other care settings with 75%-90% local travel.
• Assess members' care needs and plan personalized interventions.
• Coordinate services and monitor the care management plan against member goals.
• Assist enrollee with filing grievances and appeals.
• Provide information to enrollees about MCO requirements, services, and benefits.
