Job summary

Humana is seeking a Care Coordinator to support Illinois Medicaid Members in the Humana Gold Plus Integrated program. You will assess needs, develop care plans, and connect families with resources to improve health and well-being.

The role involves 75-90% travel, working with MLTSS or LTSS based on business needs, and remote work with occasional office travel for training. Requires Illinois residency and healthcare/case management experience.

Qualifications

• Must reside in Illinois and have 1+ years of health care or case management experience.
• 1+ years of experience with Medicare & Medicaid recipients and/or long-term care / HCBS or managed care.
• Intermediate to advanced computer skills and familiarity with EMR systems.

Responsibilities

• Visit Medicaid members in homes or care settings with 75%-90% local travel.
• Assess needs and develop personalized care plans with resources and support.
• Coordinate services and monitor the care management plan against member goals.
• Assist enrollee with grievances, appeals, and accessing benefits.
• Provide information on MCO requirements, services, and community resources.
