Job summary

Elevance Health seeks a LTSS Service Coordinator Clinician in St. Joseph County, IN. This field-based role involves traveling to member sites, coordinating care with RNs and the health team, and ensuring timely care plans and utilization of benefits.

Required licensure (LPN/LVN or LSW/LCSW/LMSW) and at least 2 years’ experience with chronic illnesses are essential. Hybrid/office attendance details will be discussed during hiring, with vaccination expectations noted.

Qualifications

• LPN/LVN, LSW, LCSW, or LMSW license in state as required.
• Minimum 2 years of experience with chronic illnesses in a service coordinator or similar role.
• Current, unrestricted licensure as applicable in state(s).
• May require state-specific certification per state law/contract.

Responsibilities

• Assist RN in identifying members for high-risk complications.
• Obtain clinical data as directed by the RN.
• Identify members who would benefit from alternative levels of care or waiver programs.
• Provide collected information to the RN for care plan development, monitoring, and evaluation.
• Coordinate care for members with chronic illnesses or disabilities with the health care team.
• Make decisions based on member needs with understanding of disease processes and clinical guidelines.
