Job summary

Elevance Health is seeking a LTSS Service Coordinator – Clinician (LSW, LCSW, LPN) for Western Ohio. This field-based role involves traveling to client sites and collaborating with RNs to assess and coordinate long-term services and supports, addressing physical health, behavioral health, and social needs.

Qualified candidates must hold an LPN/LVN, LSW, LCSW, or LMSW license (or RN alternative per state law) with at least 2 years of related experience. Hybrid requirements and local travel apply.

Qualifications

• Requires an LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with applicable state law and Nursing Diploma or AS in Nursing or a related field and minimum of 2 years of experience in working with individuals with chronic illnesses, co‑morbidities, and/or disabilities in a Service Coordinator or similar role; or any combination of education and experience, which would provide an equivalent background.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
• Strong preference for case management experience with older adults or individuals with disabilities. BA/BS in Health/Nursing preferred.

Responsibilities

• The LTSS Service Coordinator‑Clinician is responsible for the member's case and coordinates care for members with chronic illnesses, co‑morbidities, and/or disabilities under RN guidance.
• Performs activities within licensure scope including telephonic or face‑to‑face assessments for identification, evaluation, coordination and management of member's needs.
