Qualifications

• Requires an LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with state law and Nursing Diploma or AS in Nursing or a related field, plus minimum 2 years' experience.
• Current, unrestricted license in applicable state(s) required.

Responsibilities

• Identify members at high risk and collect clinical data.
• Identify candidates for alternative care levels or waiver programs.
• Coordinate care with RN and health care team to ensure efficient use of benefits.
• Support care plans and monitor member needs across physical, behavioral health, and social services.
