Elevance Health is seeking an LTSS Service Coordinator RN Clinician to oversee members' care in Brooklyn and Manhattan. The role requires clinical assessments, care plan development, and collaboration with non-RN staff to coordinate services across health, social, and long-term supports.

The position is field-based with occasional office attendance, offering flexible work arrangements within reasonable commuting distance. A valid RN license and relevant experience are required.

Formación

• HS diploma or equivalent and minimum of 3 years in coordinating care for individuals with chronic illnesses or disabilities.
• Current, unrestricted RN license in applicable state(s) required.
• May require state-specified certification based on state law and/or contract.

Responsabilidades

• Identify members for high risk, coordinate care with health team.
• Manage members with chronic illnesses to optimize health benefits.
• Develop individual care plans with member and caregivers.
