Job summary

Qualifications

• HS diploma or equivalent.
• Minimum of 3 years of experience working with individuals with chronic illnesses and/or disabilities in a Service Coordinator or Case Management role.
• Current, active RN license in applicable state(s) required.
• State-specific certification may be required by law or contract.

Responsibilities

• Identify high-risk members and coordinate care with the health team.
• Manage members with chronic illnesses to ensure cost-effective utilization of health benefits.
• Develop individual care plans with member, caregivers, and physicians.
• Perform telephonic or face-to-face clinical assessments to identify and manage needs.
• Oversee non-RN clinicians participating in the member's case and coordinate services.
