Job summary

Elevance Health LTSS Service Coordinator (Bilingual in Spanish/English) coordinates services for individuals in specialized programs. The role travels to client sites across designated Florida counties and collaborates with care teams to support PH/BH/LTSS needs.

The position requires a BA/BS degree and at least 2 years of experience in social work or related fields; bilingual English/Spanish strongly preferred, with flexibility for field visits and occasional office attendance as needed.

Qualifications

• BA/BS degree required.
• Minimum of 2 years of experience working with a social work agency or equivalent.

Responsibilities

• Conduct face-to-face program assessments using predefined questions.
• Coordinate waiver cases (LTSS/IDD) and BH/PH needs with clinical and interdisciplinary teams.
• Document member goals in the Person-Centered Plan and backup plan.
• Identify members who may benefit from alternative levels of service or other waivers.
• Assist in training and process improvement initiatives.
• Submit utilization/authorization requests with supporting documentation.
• Report critical incidents to state and county agencies.
• Support member grievances, appeals, and state audits.
