Job summary

Elevance Health in Columbus, OH is seeking a LTSS Service Coordinator-Clinician to perform field-based care coordination across physical health, behavioral health, long term services, and psychosocial needs. Assessments may be telephonic or in person, with licensure scope and RN supervision as required.

Minimum requirements include an LPN/LVN, LSW, LCSW, or LMSW license (as allowed by state law) and at least 2 years of experience with chronically ill or disabled individuals in a service

Qualifications

• Requires LPN/LVN, LSW, LCSW, LMSW or license other than RN as allowed by state law
• Minimum of 2 years working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a service coordination or similar role
• BA/BS in Health/Nursing preferred

Responsibilities

• Assists in identifying high risk members and obtaining clinical data
• Identifies members who could benefit from alternative level of care or waiver programs
• Provides information to RN who finalizes assessments and develops, monitors and revises care plans
• Coordinates care for members with chronic illnesses with the health care team for cost-effective utilization
• Under RN guidance, applies clinical guidelines and interprets disease processes
