Job summary

Elevance Health is seeking a LTSS Service Coordinator-Clinician in Columbus, OH. The role travels to client sites to coordinate long-term services, physical and behavioral health needs, with hybrid/virtual work considerations.

The clinician performs licensure-based activities, conducts assessments, and collaborates with RNs and the health care team to ensure effective, cost-efficient care for members with chronic conditions and disabilities.

Qualifications

• Requires licensure with a relevant state license (LPN/LVN or social work).
• Minimum 2 years of experience with chronic illnesses or disabilities in a coordinator role.
• Ability to perform assessments and coordinate care across health services.

Responsibilities

• Identify members for high risk complications and obtain data.
• Coordinate care with RN and health team to optimize care plans.
• Develop, monitor and revise member care plans as needed.
