Job summary

Elevance Health seeks a LTSS Service Coordinator-Clinician to provide field-based care coordination for members across specified Ohio counties. The role involves assessing physical and behavioral health, social services, and long-term supports, with occasional office attendance as needed for meetings or training.

The Clinician will perform licensure-permitted activities, including telephonic or face-to-face assessments, and collaborate with RNs to develop, monitor, and revise member care plans.

Qualifications

• Requires LPN/LVN, LSW, LCSW, or LMSW or license other than RN with minimum 2 years of chronic illness/disability experience.
• Current, unrestricted license in applicable state(s) required.
• Experience coordinating care for individuals with chronic illnesses or disabilities.

Responsibilities

• Identify members for high risk complications and obtain clinical data.
• Identify members who may need alternative levels of care or waivers.
• Provide collected data to the RN for verification, assessment, and care plan development.
• Coordinate care for members with chronic illnesses to ensure cost-effective use of health benefits.
• Make decisions based on current member needs and clinical guidelines, not nursing judgment.
