Job summary

Elevance Health in Ohio seeks an LTSS Service Coordinator-Clinician to support members across physical health, behavioral health and long-term services, coordinating care, assessing needs, and aligning resources.

This field-based role requires travel to client sites, collaboration with RNs to develop and monitor care plans, and adherence to state licensure; the hybrid/virtual work policy applies and applicants must be within commuting distance unless an accommodation is granted.

Qualifications

• Requires an LPN/LVN, LSW, LCSW or LMSW or license other than RN per state law.
• Minimum 2 years experience with chronic illnesses, co-morbidities or disabilities in a service coordination role.
• Current, unrestricted license in applicable state(s) required.
• BA/BS in Health or Nursing preferred.

Responsibilities

• Identify members with high risk needs and obtain clinical data.
• Identify members who may benefit from alternative levels of care or waivers.
• Collaborate with RN to develop, monitor, and revise care plans.
• Coordinate care with health team to ensure cost-effective utilization.
• Apply clinical guidelines based on member needs; nursing judgment not required.
