Job summary

Elevance Health is seeking a LTSS Service Coordinator-Clinician in Ohio to deliver person-centered LTSS coordination across physical, behavioral health and social needs. This role involves field work, assessing members in their environments, and coordinating care with a supervising RN and health team.

The position requires licensure (LPN/LVN, LCSW, LMSW, or similar) and prior work with chronic illnesses or disabilities.

Qualifications

• Requires LPN/LVN, LSW, LCSW, LMSW or license other than RN per state law and Nursing Diploma or AS in Nursing or related field with minimum 2 years of experience.
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Assist in identifying members for high risk complications and collect clinical data.
• Assist in identifying members who would benefit from alternative levels of care or waivers.
• Coordinate with RN to develop and monitor care plans tailored to physical and behavioral health needs.
• Collaborate with health care team to ensure cost-efficient use of benefits and appropriate service levels.
• Apply clinical guidelines in decision making while leveraging licensure scope.
