Job summary

The role requires licensure in LPN/LVN or social work, at least 2 years of experience with chronic conditions, and the ability to travel to client sites while maintaining compliance with state laws and program guidelines.

Qualifications

• Requires LPN/LVN, LSW, LCSW, LMSW or RN-eligible licensure per state law.
• Minimum 2 years' experience coordinating care for individuals with chronic illnesses or disabilities.
• Strong case management and communication skills; ability to work field-based.
• Hybrid work model with some in-office requirements.

Responsibilities

• Identify members for high risk complications and gather clinical data.
• Coordinate care with RN and health care team to ensure cost-effective utilization.
• Assess needs via telephonic or face-to-face evaluations and update the care plan.
• Support in identifying alternative levels of care or waiver programs.
