Job summary

Elevance Health seeks a LTSS Service Coordinator-Clinician to perform field-based care coordination for members with physical and behavioral health needs. The role involves assessments, care planning, and collaboration with the RN and health team across multiple Ohio counties including Butler, Warren, Clinton, and Montgomery.

The Clinician will identify high-risk members, coordinate care with chronic conditions, and support cost-effective use of health benefits while adhering to licensure

Qualifications

• Requires LPN/LVN, LSW, LCSW, LMSW or other licensed background as per state law.
• Minimum 2 years working with individuals with chronic illnesses or disabilities in a service coordination role.
• Preferred: case management experience with older adults or individuals with disabilities.

Responsibilities

• Identify members at high risk and gather clinical data.
• Identify members who may need alternative levels of care or waivers.
• Collaborate with RN to develop and monitor care plans.
• Coordinate care with the health team to ensure efficient use of benefits.
• Apply clinical guidelines within licensure scope; nursing judgment not required.
